Facilities Division’s Transportation Services has updated the Transportation Work Request process to strengthen safety compliance and improve operational efficiency across the Lab. As part of this update, the Transportation Authorization Form (TAF) has been replaced. Going forward, all requests should be submitted as online work orders through the Berkeley Lab Work Request Site

The updated process ensures hazardous materials are properly vetted before transport and aligns with the Environment, Health, and Safety (EHS) Division’s decommissioning and decontamination guidelines, reflecting the Lab’s ongoing commitment to a strong safety culture. 

Below is an overview of the changes. Note that photos of all items are required for every submission.

  • Excess/Salvage: Items going to Excess/Salvage must be approved by the relevant Safety Coordinator or Building Manager. Once it has been deemed free of hazards and all approvals are in place, Transportation Services will pick up the material or equipment and transport it to Excess/Salvage.
  • Other Material Movement: Transportation Services will move materials and equipment around the Lab, to off-site Lab buildings, and to local locations, such as LLNL, Sandia, and SLAC, with a valid Project ID (PID). A minimum of three days’ notice is required, and photos of all items must be included in the work request to support proper planning. 
  • Other Delivery Requests: Equipment and supplies, including dry ice, machine carts, metal hoppers, and shred bins, can be requested with a valid PID through a streamlined form designed for quick fulfillment. 

After submitting your work request, watch for a confirmation email. Please print and retain the specific form provided upon arrival.
